Ceftriaxone is a third-generation cephalosporin antibiotic which is often used in the treatment of bacterial infections in clinical practice. Anaphylaxis is a rare complication of ceftriaxone. A 10-year-old boy who has allergic asthma history, was prescribed intramuscular ceftriaxone. Anaphylactic shock developed in the patient after the drug was administered in the primary health care center. The patient died after ten days of intensive care unit follow-up. In this study, we presented this case in the light of the literature with pathologic and toxicological examinations performed following forensic autopsy. As a result; markers such as histamine, histaminase and tryptase are considered as indicators of anaphylaxis in the blood. It has been concluded that the timely examining of these markers in patients followed for anaphylaxis is important for determining the cause of death.
