The Effectiveness of PET/CT in the Regional Evaluation of Breast Cancer

Objective: In the staging of breast cancer, Fluorine‐18 fluorodeoxyglucose positron‐ emission tomography (PET/CT) is used especially for detecting distant metastasis, as well as for evaluating regional status. Conflicting results are seen in studies evaluating both the axilla and the receptor status of the tumor in PET/CT. In this study, we aimed to perform the efficacy of PET/CT in evaluating both axilla and receptor status of the primary tumor.

Materials and Methods: Between January 2015-June 2017, seventy-five patients, diagnosed breast cancer and performed PET/CT before neoadjuvant therapy, were included in the study. All the patient’s data were analyzed retrospectively. Patients with distant metastasis, non-axillary lymph node metastasis and given neoadjuvant therapy before PET/CT were excluded from the study.

Results: While 53 of the 75 patients in the study had lymph node metastasis histopathologically, 22 of them did not. According to the PET/CT, 36 patients were positive and 39 were negative. The sensitivity of PET/CT in evaluating the axilla was 62.3%, while specificity, accuracy, positive predictive value and negative predictive value were 86.3%, 69.3%, 91.6% 48.7% respectively. No significant difference was observed between the luminal groups in terms of SUVmax. As the positivity of estrogen and progesterone receptors in the tumor was increased, SUVmax was decreasing.

Conclusion: Although the specificity of PET/CT in evaluating the axilla is high, its sensitivity was no longer found to be sufficient. In addition, although the SUVmax value of the primary tumor differed in terms of histological receptors, it is not sufficient to predict.
