Comparison of Posterior and Lateral Approaches in the Treatment of Type III Displaced Supracondylar Fractures of the Humerus in Children

The purpose of this study was to compare the efficacy of posterior and lateral approaches for the treatment of displaced supracondylar fractures of the humerus in children.

Our study involves 36 patients diagnosed with supracondylar humerus fractures and referred to emergency service of the hospital between March 2006 and February 2008. 14 patients who underwent open reduction and pinning performed with lateral incision were evaluated in the first group; and 22 patients who underwent open reduction and pinning performed with posterior incision were evaluated in the second group.

Functional result of patients in the first group were better than the results of the patients in the second group. However patients who underwent an open reduction with posterior incision had better cosmetic result.

Since minor damage was given to tissues in lateral approaches, functional results in these procedures were better, but because of the difficulty of anatomical approach cosmetic result was poor. On the other hand, posterior approach had a poor functional result as it gave much damage to tissues yet due to better control over the anatomy it had a good cosmetic result.
