Severe Hyponatremia with Primary Adrenal Insufficiency in Elderly: A Case Report and Review of Literature

Hyponatremia is the most common cause of electrolyte disorders and also a common cause of hospitalisation in older people. The causes of hyponatremia involved congestive heart failure, liver failure, hyperglycemia, using of thiazide and antipsychotics and the hormonal disorders like hypothyroidism, adrenal failure. In older people, hyponatremia seen more frequently because of the comorbid diseases and these diseases may be more complicated with the hyponatremia. Adrenal failure is often accompanying with hyponatremia, hyperpotassemia and hypotension. Tuberculosis is a common cause of adrenal failure and the mechanism of action has not been understood exactly. The symptoms and signs of adrenal insufficiency depend on the rate and extent of loss of adrenal function, and the symptoms develop because of cerebral edema. Symptoms consist of nausea, vomiting, headeache, muscle cramps, convulsion, coma. Adrenal failure can not be recognised until the adrenal crisis triggered by an illness or a stress factor. In this case report, we presented a chronic hyponatremia case with adrenal failure in an elderly patient.
