Detection of Metallo-Beta-Lactamase Enzyme in Gram Negative Non-Fermantative Bacteria by Different Methods

Objective: In this study, we aimed to investigate the production of metallo-beta-lactamase (MBL) at clinical isolates resistant to meropenem and to compare phenotypic methods used for detection of these enzymes.

Material and Methods: The identification of bacteria was made with standard clinical microbiological methods and confirmed in the BD Phoenix fully automated identification system. In vitro bacterial resistance to antibiotics was determined by using disc diffusion method in accordance with the recommendations of The Clinical and Laboratory Standards Institute (CLSI) and confirmed with the BD Phoenix fully automated identification device. Production of MBL were detected by combined disc diffusion method, double-disk synergy test, modified Hodge test and E test methods.

Results: The MBL production within the meropenem resistant Acinetobacter strains were found to be 95% with the combined disc method, 67.5% with MP-EDTA double-disc synergy test, 72.5% with the modified Hodge test and 97.5% with E test. MBL production within meropenem resistant Pseudomonas strains were 80% with the combined disc method, 100% with MP-EDTA double-disc synergy test, 60% with the modified Hodge test and 60% with the E test.

Conclusion: The results of this study suggests that a single phenotypic tests is inadequate for the identification of MBL. Furthermore, more extensive studies such as sequencing are required for the identification and control of MBL producing strains.
