Anxiety, Depression, Perceived Stress Levels, Eating Attitude and Sleep Quality in Chronic Tension Headache Patients

Objective: Tension-type headache (TTH) is the most common primary headache and in this study, we aimed to examine anxiety and depression levels, perceived stress level, eating attitudes and sleep quality and possible relationships between them in patients with TTH.

Materials and Methods: 43 patients and 43 healthy controls were included in the study. Sociodemographic and Clinical Data Form, Perceived Stress Inventory (PSS), Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Eating Attitude Test (EAT), Pittsburgh Sleep Quality Index (PUKI) were administered to the participants.

Results: The PSS (p<0.001), BDI (p<0.001), BAI (p<0.001), EAT (p=0.041) and PUKI (p<0.001) scores of the patient group were found to be significantly higher than the control group. A significant positive correlation was found between EAT score and PUKI, PSS, BDI, BAI and the number of attacks, another significant positive correlation was found between PUKI and PSS, BDI, BAI and the number of attacks, and a significant positive correlation between PSS and BDI, BAI, number of attacks was found. The number of attacks per year predicted the EAT score (β=.084, p=0.0151).

Conclusion: The patients' anxiety, depression, and perceived stress levels were found to be significantly higher than the control group, and their eating attitudes and sleep quality were found to be significantly worse. The number of headache attacks experienced by the patients predicted their eating attitudes. We think that patients may turn to unhealthy eating behaviors in order to cope with pain, and identifying comorbid psychiatric diseases in these patients can shape the clinical course, prognosis and treatment strategies of headache.
