Evaluation of Our Papilledema Patients With Visual Field, Pattern VEP, Ishihara, Farnsworth-Munsell 100 Hue Color Discrimination Tests

Objective: Papilledema is optic disk swelling caused by raised intracranial pressure. The aim of this study is to share etiological evaluation and follow up results of our papilledema patients with, visual field, visual evoked potentials (VEP) and color discrimination tests.

Materials and Methods: Sixteen patients with papilledema were included in the study. Visual Field test was performed with Humphrey’s field analyser. Color vision was evaluated with Ishihara and Farnsworth-Munsell (FM) 100 Hue test. Total error scores and error scores of each box of FM 100 Hue test were compared with control group by Mann Whitney U test. Pattern VEP was performed in the neurology clinic.

Results: 11 (68.75%) of the patients were female, 5 (31.25%) were male. Mean age was 38.8±9.2 years. One of the patients had hydrocephalus, 2 had intracranial mass, 3 had neurobrucellosis,1 had neurobehcet, 1 venous sinus thrombosis. Remaining 8 patients were diagnosed as Idiopathic Intracranial Hypertension (IIH) according to modified Dandy criteria. All of the patients diagnosed with IIH were obese (BMI>28) and female. The most frequent visual field defects were blind spot enlargement and constriction of peripheral field. P100 latancies of VEP tests were between 89.2-122 (mean 100.87±8.47). The difference between total error scores compared with control group, was statistically significant (p=0.043).

Conclusion: If papilledema is diagnosed early, visual acuity, color vision with Ishihara cards and VEP is not affected, while in chronic papilledema stage, the disease acts like an optic neuropathy, where, visual acuity decreases, advanced visual field defects become permanent, detoriation at color vision tests and defects in VEP could be seen.
