Retrospective Analysis of Demographic and Clinical Characteristics of Patients with Pemphigus

Objective: Pemphigus is an autoimmune bullous disease characterized by acantholysis, bullae and erosions in the mucosa and the skin. In this study, it was aimed to examine the demographic and clinical characteristics of 56 patients with pemphigus retrospectively.

Materials and Methods: In the study 56 patients were included between April 2013-April 2023. Pemphigus Disease Area Index (PDAI) was used to determine disease severity. Demographic, clinical and laboratory findings of the patients were obtained from hospital database.

Results: In this study, 50 (89.3%) patients with pemphigus vulgaris (PV) and 6 (10.7%) patients with pemphigus foliaceus (PF) were evaluated. Of the patients, 29 (51.8%) were female, 27 (48.2%) were male, and the mean age was 52.66±14.96 years. Of the patients with PV, 17 (30.4%) were oral mucosal type and 33 (58.9%) were mucocutaneous type. Ocular involvement was found to be significantly higher in men, and urinary infection was found to be significantly higher in women (p<0.05). Ocular involvement, hypertension (HT), hyperlipidemia, pulmonary embolism and benign prostatic hypertrophy were found to be statistically significantly higher in patients over 60 years of age (p=0.019, p=0.006, p=0.001, p=0.026, p=0.026, respectively). The use of rituximab in severe and very severe disease was found to be significantly higher than the use of systemic corticosteroids (p=0.000).

Conclusion: Osteoporosis, HT, and hyperlipidemia were found to be high in patients with pemphigus. When planning the treatment the accompanying diseases and the additional risks of systemic corticosteroid therapy should be revealed and the patients should be followed carefully.
