Examination of the Frequency of Cryptorchidism, the Age of Orchidopexy, and Additional Inguinal Anomalies in Adult Testicular Tumors

Amaç: Bu çalışmada; TT tespit edilmiş olgularda, kriptorşidizm sıklığı, orşiopeksi yaşı ve ek inguinal anomalilerin etkisinin araştırılması amaçlandı. Gereç ve Yöntem: 2013-2023 yılları arasında üroloji kliniğinde TT nedeniyle tedavisi yapılan olgular retrospektif olarak incelendi. Olguların yaşı, hastaneye başvuru şikayeti, tümörün tarafı, tipi, kriptorşidizm varlığı ve tarafı, orşiopeksi yaşı, gelişen TT tipi ve ek inguinal anomalilerin varlığı araştırıldı. Bulgular: TT nedeniyle tedavi edilen 94 erkek olgunun median yaşı 29.5 yıldı. En sık başvuru şikayetleri; testiste ağrı, şişlik ve ele gelen kitleydi. TT olguların %50'sinde sol, %47.9'unda sağ ve %2.1'inde bilateral yerleşimliydi. Olguların %94.7'sinde malign TT (%45.8 seminom), %5.3'ünde ise benign TT gelişmişti. Olguların 8'inde kriptorşidizm mevcuttu, 3'ünde orşiopeksi öyküsü yoktu ve median yaşı 26.5 yıldı. Orşiopeksi yaşı median 3 yıldı. Kriptorşidizmli olguların %50'si sol, %37.5'i sağ, %12.5'i bilateraldi ve hepsinde malign TT gelişmişti. Tüm olguların %44.7'sinde ek inguinal patoloji mevcuttu. Kriptorşidizm öyküsü olanlarla, olmayanlar arasında TT gelişme yaşı ve tümör tipi açısından istatistiksel olarak anlamlı farklılık yoktu (p=0.770, p=0.326). Tümör gelişen taraf açısından ise istatistiksel olarak anlamlı farklılık vardı (p=0.003). Sonuç: Kriptorşidizmli hastalara TT riski nedeniyle erken dönemde orşiopeksi yapılmalıdır. Orşiopeksi, testis tümörü riskini tam olarak ortadan kaldırmamakla birlikte hem fizik muayene hem de görüntüleme yöntemleri ile takibi kolaylaştırır. 20 yaşından sonra görülen hidrosellerde TT olabileceği bilgisi unutulmamalıdır.
Objective: In this study; it was aimed to investigate the frequency of cryptorchidism, the age of orchidopexy and the effect of additional inguinal anomalies in cases with testicular tumor (TT). Materials and Methods: Cases treated for TT between 2013-2023 were retrospectively examined. The age of the cases, the complaint by admission to the hospital, the side and type of the tumor, the presence and side of cryptorchidism, the age of orchidopexy, the type of TT developed and the presence of additional inguinal abnormalities were investigated. Results: The median age of 94 male cases treated for TT was 29.5 years. The most common complaints by application were pain, swelling and a palpable mass in the testicle. TT was located on the left in 50% of the cases, on the right in 47.9%, and bilaterally in 2.1%. Malignant TT occurred in 94.7% of the cases (45.8% seminoma), and benign TT occurred in 5.3%. Cryptorchidism was present in 8 of the cases, there was no history of orchidopexy in 3, and median age was 26.5 years. The median age at orchidopexy was 3 years. Of the cases with cryptorchidism, 50% were left, 37.5% were right, and 12.5% were bilateral. Malignant TT developed in all cases with cryptorchidism. Additional inguinal pathology was present in 44.7% of all cases. There was no statistically significant difference in terms of age at development of TT and tumor type between those with a history of undescended testicles and those without (p=0.770, p=0.326). There was a statistically significant difference in terms of the side where the tumor developed (p=0.003). Conclusions: Patients with cryptorchidism should undergo early orchidopexy due to the risk of TT. Although orchiopexy does not eliminate the risk of testicular tumor, it facilitates follow-up with both physical examination and imaging methods. It should not be forgotten that hydroceles seen after the age of 20 may be TT.
