Lenfatik akım konnektif dokuda ki yalnız endotel tabakası içeren lenfatik kapillerlerden başladıktan sonra anatomik pleksuslara ve ortak lenfatik kanallara doğru devam eder. El, ön kol ve kolun fasia tarafından bölünmüş yüzeyel ve derin lenfatik sistemleri vardır. İki sistemin lenfatik akımı aksillada birleşir ve buradan subklavian lenfatik trunkus tarafından, Jugulo-subklavian birleşim yerinde venöz sisteme drene olur.
Lenfödem, deri ve deri altı dokunun hipertrofisi ve kronik inflamatuar fibrozisi ile karakterize progresif patolojik bir durumdur. Primer ve sekonder lenfödem olarak ikiye ayrılır. Sekonder lenfödemin en sık nedeni meme kanseri tedavisinden sonra kolda görülen lenfödemdir. Yapılan çalışmalarda mastektomi sonrası gelişen lenfödemin fizyopatolojisi net olarak ortaya konulamamıştır. Cerrahi tedavi ve/veya radyoterapiye bağlı lenfatik sistemin hasarlanmasının patogenezde önemli rol oynadığı düşünülmektedir. Bu hasardan çoğunlukla cerrahi olarak lenfatik damarlarının ligasyonu, transeksiyonu ve rezeksiyonu ile radyasyona bağlı lenfatiklerde, lenf nodlarında veya çevre dokuda gelişen fibrozis sorumludur. Ayrıca cerrahi ve radyasyon tedavisi dışında enfeksiyonlar, venöz ve arteriel nedenler gibi tam açıklanamayan faktörlerde fizyopatolojide rol almaktadır.
Lymphatic flow begins lymphatic capillars which is consisting only endothelial layer in the connective tissue, and goes through to the anatomic plexus and common lymphatic channels. There are superficial and deep lymphatic systems that is seperated by fascia in hand, arm and forearm. This two lymphatic systems join in the axilla and go through to the jugulo-subclavian venous junction by subclavian lymphatic vessel and also drain into the venous system.
Lymphedema is hypertrophy of the dermis and hypodermis and progressive pathologic conditions which is characterized chronic inflammatory fibrosis. There are two types of lymphedema which are primary and secondary causes. The most common cause of secondary lymphedema is in the arm which is seen after breast cancer therapy. Pathophysiology of the lymphedema after mastectomy is not clearly understood. It is thought that lympathic system could be damaged after surgical- and/or radio-therapy. Surgical ligations, transsections, and resections of lymphatic vessels, fibrozis of the lymphatics, lymph nodes and surrounding tissue due to radiation are responsible from this damage. However, infections, and unexplained factors like venous and arterial reasons are also responsible in the pathophysiology.
